CBSI Neurosurgical Auto Accident Checklist

If your appointment involves an automobile accident claim, please bring all of the items listed below:

  • The name, address, and telephone number of your automobile insurance carrier, along with verification that you have medical payments coverage on your policy.
  • The claim number and date of the accident.
  • If an attorney is involved, please bring the attorney's name, address, and telephone number.
  • The name and telephone number of your claim adjuster.
  • Insurance cards with Group Number, Member ID, and Payer ID.
  • Primary Care Referral letter if required by your insurance.
  • All x-ray films performed which are pertinent.
  • Reports and medical records which are pertinent.
  • A written list of questions you have for our physician.
