If your appointment involves a worker's compensation case, please bring all of the items listed below:

  • The name, address, and telephone number of your employer at the time of the injury.
  • The name, address, and telephone number of your employer's work compensation carrier.
  • Your claim number and the date of your injury.
  • The name and telephone number of your adjuster.
  • If an attorney is involved, please bring the attorney's name, address, and telephone number.
  • Insurance cards with Group Number, Member ID, and Payer ID.
  • Primary Care Referral letter if required by your insurance.
  • All x-ray films performed which are pertinent.
  • Reports and medical records which are pertinent.
  • A written list of questions you have for our physician.
