If you cannot keep your scheduled appointment, please contact our office at least 24 hours prior to your appointment to cancel or to reschedule.

First Appointment Checklist
Please come prepared for your first appointment and remember to bring the following:

• Insurance cards with Group Number, Member ID, and Payer ID.
• A Primary Care Referral letter if required by your insurance.
• All radiographic/diagnostic studies including X-rays, MRI, CT, EMG/NCV, disography, etc. Please bring radiographic reports for all test results.
• Reports and medical records which are pertinent.
• Please bring original signed patient history, demographics and practice policy forms.
• A written list of questions you have for our physician.

X-RAYS and RADIOGRAPHIC Studies:

A very important aspect of your appointments with our Neurosurgeons is the process of reviewing all pertinent x-rays and radiographic film such as CAT scans and MRIs. Physical hard copies, ICDs and original CDs (not a copy) are preferred. X-rays in particular can not be faxed. It is your responsibility to bring these files to your appointment. With this in mind, we recommend that you allow time to request and receive these records prior to your appointment. Without physical copies of your films, your appointment may need to be rescheduled.
